Do not sign a Form 1583 that contains incorrect information. Form 1583 is a federal form; signing it with a wrong address or name creates a compliance problem that is harder to unwind than a correction.
Your updated form is pre-filled from the details already on your account. If those details are out of date, the new form carries the old information forward.
What customers most often find wrong
A home address you have moved away from. The form shows the residential address on file, not your current one. If you have moved, your address needs updating on the account before the form is regenerated.
A misspelled or changed name. Including names that changed through marriage or divorce.
The wrong mailbox-open date. Some long-tenured accounts show a date later than when the mailbox was actually opened.
A recipient who should no longer be on the mailbox. Someone who has moved out, left the business, or passed away.
What to do
Reply to your Form 1583 email or open support chat and tell us:
Which recipient's form is affected.
Which field is wrong.
The correct value.
We update the account record and regenerate the form. You will get a new signature request once it is corrected. Your ID documents do not need re-uploading unless the correction is to your address — in which case your address ID must match the new address.
If you have moved
Update your home address first, then ask for the form to be regenerated. If your new driver's licence or lease shows the new address, upload it as your address ID at the same time so both match.
Does this pause my deadline?
Raise it as soon as you notice. Your 30-day window keeps running while a correction is in progress, so tell us if your deadline is close and we will flag the account.
